I think that you may have to replace these two lines in your control file:
     10.  # No. of data items
  5000.000 0.000 5000.000 0.000 1.000 3.000 -5.000 1.000 0.000 0.000

with these two:
     12.  # No. of data items
  5000.000 0.000 5000.000 0.000 1.000 3.000 -5.000 1.000 0.000 0.000 1.000 
0.000


using 12 instead of 10 data items worked for me. I am not familiar with the 
differences between all these control data items for this control law. I am 
sure it could be better re-structured (..I doubt though that you will find 
many volunteers for this).
